Membership in the National Forum for Heart Disease and Stroke
Prevention is comprised of two member categories: Organizational Members and Individual
Members. Both member categories are
required to complete a membership application form.
The Membership Committee reviews the applications and makes recommendations
to the Coordinating
Board for confirmation.
1) Organizational
members provide expertise, support, collaborative leadership and participation
in joint actions that impact the recommendations in A Public Health Action
Plan to Prevent Heart Disease and Stroke
(Action Plan). The organization’s staff and volunteers,
as member delegates, help implement, expand and champion National Forum projects
that address heart disease and stroke prevention efforts. Organizational members
are committed to providing resources and forming effective and beneficial partnerships
with the National Forum. Organizational Members may actively engage in the
work of the National Forum by participating on Implementation Groups and Committees.
2) Individual
Members provide expertise, support, and participation in joint actions that
impact the recommendations in A Public Health Action Plan to Prevent Heart
Disease and Stroke.
