Heart disease and stroke are among the nation’s leading causes of death and major causes of disability, costing our nation hundreds of billions of dollars annually. In the next two decades, these conditions are expected to increase sharply as the country’s ‘baby boom’ generation ages. Fortunately, these conditions are largely preventable. To reverse this epidemic of heart disease and stroke through increasingly effective prevention, action is needed now.

The National Forum for Heart Disease and Stroke Prevention is a partnership of more than 80 organizations committed to building a heart healthy stroke free society. The National Forum provides national leadership and encourages collaboration for the implementation of A Public Health Action Plan to Prevent Heart Disease and Stroke--a comprehensive public health strategy to chart the course for the prevention of heart disease and stroke. The Action Plan was first published in 2003 and was updated by the National Forum in the spring of 2008.
