MISSION: To monitor the burden of heart disease and stroke and measure progress in the prevention and treatment of heart disease and stroke by:
• Expanding and standardizing population-wide data sources and activities
• Establishing frameworks, methods, and core indicators for evaluation of policy, environmental and systems change interventions
• Developing professional staff capacity for monitoring and evaluation

Current Activities
1) Continue to work closely with the APIG to develop a specific National Forum
advocacy strategy relative to the establishment of a national HDS Surveillance Unit.
2) Co-present the advocacy strategy with the APID to the Coordinating Board for their
review and approval.
3) Get the article on evaluation of policy and environmental and systems
change for HDS published in a reputable national journal.
4) Disseminate the evaluation article and its recommendations to National Forum
members and partners, and other appropriate audiences.
5) Develop and implement effective ways to assist state-based HDSP programs in
identifying resources for program evaluation.
6) Complete the Capacity Project Team white paper on assessing and
improving HDS surveillance and evaluation capacity and competencies, and
present it to the Executive Committee for approval.
