2010 National Forum Awards
The National Forum is seeking nominations for National Forum Awards to be presented at the 8th National Forum from for the following awards:
National Forum Heart Healthy Stroke Free Award
The National Forum Heart Healthy Stroke Free Award is presented to an individual and group in the field of public health whose work embodies the recommendations of the A Public Health Action Plan to Prevent Heart Disease and Stroke. This award is presented during the annual meeting of the National Forum.
National Forum Public Policy Award
The National Forum Public Policy Award is given to an individual and group that have been effective in promoting policy change that reduces the heart disease and stroke burden on a national, state, or local level. This award is presented during the annual meeting of the National Forum.
National Forum Commitment Award
The National Forum Commitment Award is given to an individual who has made outstanding contributions to the National Forum.
